Every program we offer requires a Liability Release and Registration Form. Some programs require a supplemental form as well. These forms are kept on file for one year and need to be updated every January 1st. You can get these forms online here:
Payment Methods Accepted
McKinleyville Parks and Recreation Department accepts cash, money orders, checks, debit cards and Visa, MasterCard or Discover credit cards. Please make checks payable to MCSD.

The correct fee and a completed registration form must be included. Please do not mail cash. Make checks payable to: MCSD Mail form and fee to P.O. Box 2037 McKinleyville, CA 95519.
Pre-registration is required, prior to the first meeting, and full payment is required for all activities, unless other arrangements are made.
Fee Reduction Fund
A $20.00 fee will be charged for returned checks in addition to the check amount.
Classes and activities may be cancelled 2-5 days before the first scheduled meeting due to insufficient enrollment. Register early to assure your choice.
In the event your desired class is closed, please be sure to request that your name be added to an Interested List. This enables McKinleyville Parks and Recreation to track interest and include those interested in new and future classes.
Full refunds will be issued for any class or activity cancelled by McKinleyville Parks and Recreation. Refunds issued may take up to 4 weeks to be received in the mail.
Refunds for other reasons must be requested by completing a Request for Refund form. A $10.00 service fee per enrollment may be assessed.
McKinleyville Parks and Recreation is not permitted to issue cash for refund requests.
